Funding pathways from IHN-CCO, upcoming information sessions

InterCommunity Health Network CCO and the Delivery Systems Transformation Committee invite interested community partners in Benton, Lincoln, and Linn counties who can positively impact the health and wellbeing of IHN-CCO members to submit pilot proposals that transform the healthcare delivery system or to submit proposals to support DST committee engagement and small-scale project work as new partners to the DST.

All proposals must address one or more priority areas of the Community Health Improvement Plan:

  • Access to Affordable Housing
  • Access to Quality Care
  • Behavioral Health
  • Inclusion, Diversity, Anti-Racism & Equity (IDARE)

Applications are due July 15, 2025 at noon.

2025 Funding Pathways

  • New Partner Engagement Funds: The intention of this funding opportunity is to invite new partners to get to know the DST, participate in meetings and workgroups, and provide some additional support to carry out project work.
  • Pilot Project Funds: Pilot projects should either be a new idea or bring in an existing idea that would be new to our region, or carried out in a new, creative way. Pilot Project Funds are available for proposals with a budget of greater than $15,000 that are focused on innovation and system transformation.
