IHN-CCO and the Delivery System Transformation Committee (DST) invite community partners in Benton, Lincoln, and Linn counties to submit pilot proposals that transform the healthcare delivery system or to submit proposals to support DST committee engagement for new partners.
2026 Funding Pathways
- New Partner Engagement Funding: Invitation for new partners to participate in in the DST and provide support for small-scall projects
- Pilot Project Funding: Opportunity for larger-scale projects that are focused on innovation and system transformation
