Updated County Health Rankings and Roadmaps

County Health Rankings & Roadmaps (CHR&R), a program of the University of Wisconsin Population Health Institute, draws attention to why there are differences in health within and across communities. The program highlights policies and practices that can help everyone be as healthy as possible.

The 2025 Annual Data Release debuts a bold new way of broadening our understanding of health along with updated data and tools. The resources are designed to help improve health and equity, so everyone has what they need to live long and live well. Click here to learn about the updated data model and how to look at key health indicators in Oregon and the nation.
